Questions Asked During Visit
When you are brought into a client room with your pet, our staff will ask you a series of questions that the Doctor will review prior to the exam. The questions will differ depending on what you are bringing your pet in for. We have provided these questions to prepare you ahead of time, and in case the person who knows all the answers is unable to make it to the appointment! These are PDF documents and can be printed out. Please fill out the form that most closely matches the reason you are coming in with your pet and bring it in with you.
